DMCA Policy

Respect for intellectual property rights is foundational to the mission of the Pennsylvania Forms website. We are committed to complying with U.S. copyright law and to supporting the rights of content creators and copyright owners. If you believe that your copyright has been infringed upon on our site, we encourage you to let us know. Our Digital Millennium Copyright Act (DMCA) policy outlines the process through which copyright complaints can be handled and resolved.

Notification of Copyright Infringement

To file a notice of infringement with us, you must provide a written communication detailing the copyrighted work that you believe has been infringed, as well as the specific location on the Pennsylvania Forms website where this material is located. Please note that to be effective, your notification must include the following:

  • A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  • Identification of the copyrighted work claimed to have been infringed, or, if multiple works at a single online site are covered by a single notification, a representative list of such works at that site.
  • Identification of the material that is claimed to be infrading or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit the service provider to locate the material.
  • Information reasonably sufficient to permit the service provider to contact the complaining party, such as an address, telephone number, and, if available, an electronic mail address at which the complaining party may be contacted.
  • A statement that the complaining party has a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

All DMCA notices should be sent to our designated agent at dmca@pennsylvaniapdf.com. We advise that you consult with your legal advisor before filing a notice. Also, be aware that there can be penalties for false claims under the DMCA.

Response to Infringement Notices

Upon receipt of a compliant notice of copyright infringement, it is our policy to remove or disable access to the infringing material, to notify the content provider, author, or user that we have removed or disabled access to the material, and that repeat offenders will have their access to the website removed. Our process is designed to ensure that copyright owners' rights are respected while also protecting the freedom of expression and avoiding unjustified takedown of lawful content.
